This form allows parishioners to:

• Indicate their intended Adult Envelope contribution amount

• Specify whether the commitment is monthly or yearly

• Optionally designate up to 25% of that contribution toward a specific parish need

Designations may be directed to:

• Capital Improvements

• Maintenance, Repairs & Utilities

• Staffing, Faith Formation, Outreach & Evangelization
